The Care Coordination Department at Lawrence Memorial Hospital includes specially trained registered nurses and licensed social workers. Our team is committed to providing support through your illness and helping you plan your transition from the hospital.
We provide a variety of care coordination services and resources for LMH patients:
- Assistance with illness counseling and financial concerns
- Coordinating patient dismissal plans
- A list of resources available in the community to support our patients' continued recovery to better health and wellness
- Information about completing an Advance Directives, if desired. An Advance Directive is a document that states an individual's health care wishes should they ever become unable to speak for themselves. A booklet describing all the details of Advance Directives, as well as an easy to fill out form, is available here.
The LMH Care Coordination Department is open Monday through Friday, 8 a.m. to 4:30 p.m. If you would like to speak to a social worker, you may ask a nurse to contact us or call our office at 785-505-6149.
