How do I make an appointment?

Call our main line at 785-505-2800 to make an appointment. A physician's referral is required for new patients. Please ask your physician's office to send your medical records to us prior to your first visit.

I'm a new patient. How should I prepare for my first visit?

The best way to come prepared for your first appointment is to complete our new patient forms in advance and bring them with you. We also recommend arriving 15 minutes early for your first visit so all your paperwork and insurance information can be processed before your scheduled appointment time. Please bring your photo ID, insurance card(s) – including your prescription insurance card if applicable – and all medicines you are currently taking in their labeled bottles (that includes prescription and over-the-counter meds, as well as vitamins and herbal supplements).

  • You have uncontrolled pain.
