It is the policy of the Center for Rehabilitation to conduct home evaluations which assess the home environment and provide recommendations which promote independent functional capacities. This assists in providing a safe transition from the controlled hospital setting into an environment which promotes maximum independence. The home evaluation is scheduled with the social worker, physical therapist, or occupational therapist. The home evaluation is conducted by either the physical or occupational therapist. The patient and family must be present during the home evaluation. Common recommendations include removal of architectural barriers, rearranging furniture to permit ease in passage, addition of adaptive equipment and general safety tips.
