Our approach to care

We are dedicated to providing each patient with a personalized rehab experience and collaborative rehab team to help each patient reach their goals. Because we are located in the main hospital, patients have access to our other hospital services, such as laboratory, pharmacy, and consultations as needed.

Our services

  • We offer all three forms of therapy: physical, occupational and speech therapy.
  • For each patient, a physiatrist, APRN, and RN will oversee acute and chronic health diagnosis while receiving therapies. Patients must be able to tolerate 3 hours of therapy a day.
  • Our pharmacist will review and help manage prescribed medications.
  • A collaborative therapy meeting is conducted each week to review the patient’s progress and goals.
  • Services provided by our social worker, dietician, medical hospitalist, psychiatric APRN and chaplain are also available.

Our Acute Inpatient Rehab unit is fully equipped with all of the amenities you need for a comfortable and successful recovery, such as:

  • Private room for each patient

Planning to return home

We want our patients to be fully prepared for their return home after visiting our clinic. The following processes are in place to prevent readmission, manage complications, and keep you healthy and safe in the community:

  • Each patient will receive daily education on how to manage acute or chronic health diagnoses at home and home discharge instruction from one of our nurses.
  • Our in-house laboratory, pharmacy and diagnostic testing will monitor stability and help prevent future conditions.
  • Upon discharge, each patient and/or their family will have one-on-one meetings with a pharmacist to manage medications, as well as patient/family training with the therapy team.
  • Follow up appointments with primary care and/or specialists involved in patients’ care, home health and outpatient rehab can be scheduled to help patients continue to manage their health conditions after discharge and prevent readmission to the hospital.
