The Transitional Care unit is designed to bridge the gap between acute hospital care and discharge to your home or long-term care facility. Our 17-bed, short-term care unit provides an environment of wellness and rehabilitation, and create a healing setting for those with an end-stage condition.
Acute care vs. Transitional care
The TCU is a short-term skilled nursing unit, located across the hall from Acute Inpatient Care on the 4th floor of the hospital on the LMH Health Main Campus. We want you to feel at home in our facility, so we take special care to ensure you’re comfortable and at ease during your rehabilitation.
Our Approach to Care
Our skilled-nursing unit is designed to provide a more personal level of care than one you might find at a larger facility. During your stay, you will receive an individualized plan of care that is reviewed weekly by our interdisciplinary care team. Our goal is to help you recover and return home to your daily routine by:
- Developing a plan of care with our nursing staff, physiatrist, therapists, restorative staff, dietician and social worker.
- Scheduling group meetings with your care team every week, as well as inviting family to take an active role in your care as well.
- Providing access to our hospitalists and other services like imaging or lab work all in one location.
