Do you accept my insurance plan?

We contract with major insurance carriers and file claims with all insurance companies. To confirm that we are contracted providers with your plan, please call your insurance carrier or visit their website. For more details, contact our billing office at 866-434-1545 ext. 85419.

How do I make an appointment?

Call our main line at 785-505-2800 to make an appointment. A physician's referral is required for new patients. Please ask your physician's office to send your medical records to us prior to your first visit.

I'm a new patient. How should I prepare for my first visit?

The best way to come prepared for your first appointment is to complete our new patient forms in advance and bring them with you.

We also recommend arriving 15 minutes early for your first visit so all your paperwork and insurance information can be processed before your scheduled appointment time. Please bring your photo ID, insurance card(s) – including your prescription insurance card if applicable – and all medicines you are currently taking in their labeled bottles (that includes prescription and over-the-counter meds, as well as vitamins and herbal supplements).

I have a question about my bill. Who should I contact?

  • For questions regarding a physician fee on your LMH Health Cancer Center bill, call the physician central billing office at 866-434-1545 ext. 85419
  • For all other billing inquiries, please call LMH Health Patient Accounts at 785-505-5775.

As a patient, what problems should I watch for and when should I contact my doctor?

If you're experiencing any of the following problems – or if you have any questions/concerns at all – please call us:

  • Your temperature is at or above 100.5°F.
  • You have uncontrolled pain.
  • Nausea or vomiting is not relieved with medications.
  • Diarrhea or constipation not relieved with medications.

I need my records sent to another provider. Is there a form I should complete?

Yes. We are required to obtain your written authorization for any use or disclosure of your protected health information (PHI). You can view and print the Authorization Form now, or you can request a copy by calling our office at 785-505-2800.
