LMH Health is committed to protecting the privacy and security of our patient’s medical records and other health information. LMH Health has entered into an agreement with other healthcare entities to protect this information. Authorized representatives of these entities may be given access to LMH Health medical records and other health information by completing the LMH EMR Remote Access Terms and Conditions for Authorized Personnel.

Authorized representatives must also read and agree to abide by the LMH Code of Conduct, the LMH Confidentiality Policy and the LMH Responsible Use Policy.  Authorized representatives must also read and review the LMH Compliance Education provided and acknowledge that they understand the Compliance Education.
