Who can register?
If you’re a patient at LMH Health or one of our affiliated clinics and are 18 or older, you can register online for My Patient Portal. Parents also can also register their children who are younger than 13. Learn more about how to register.
Connect your health management apps
Apple Health connects with My Patient Portal so you can easily access your electronic medical record (EMR). My Patient Portal may be available to connect to other apps.

TEAM is a Centers for Medicare & Medicaid Services (CMS) program that coordinates your care during and after certain surgeries—through the first 30 days of recovery—to improve outcomes and reduce avoidable readmissions.
