LMH Health is a Medicare Accountable Care Organization

LMH Health participates in a Medicare Accountable Care Organization (ACO). An ACO is a group of doctors, hospitals and healthcare providers who work together to improve care for patients.

Our organization is now participating in a Medicare program called an Accountable Care Organization (ACO) through Centrus. This program is designed to help your doctors, nurses, and care teams work more closely together to support your health.

What This Means for You

Because you have Traditional Medicare and receive primary care services from one or more providers at LMH Health, you are automatically included in this ACO. You do not need to take any action.

Being part of an ACO does not change:

• Your Medicare benefits
• Your ability to see any doctor or hospital that accepts Medicare
• Your costs or coverage
• Your freedom to choose your healthcare providers

The ACO helps your care team collaborate to provide better care by:

• Supporting and expanding preventive care opportunities
• Improving communication among your healthcare providers
• Reducing duplication of tests, unnecessary provider appointments and avoidable hospital visits

Your Rights & Choices

Being part of an ACO is voluntary. You still have the right to:

• See any doctor or hospital that accepts Medicare
• Decline to have your information shared for care coordination
• Ask questions or request more information

If you choose not to have your information shared for ACO care coordination, you can call 1‑800‑MEDICARE (1‑800‑633‑4227).

No Action Needed

You do not need to sign anything or change any of your current healthcare arrangements. This letter is simply to keep you informed of this new Medicare initiative and how it supports your care.
