Medication management services provide a structured review of all the medications you take to identify potential interactions, duplications, or opportunities for safer or more effective treatment. Clinic pharmacists work collaboratively with your primary care provider to help you manage multiple prescriptions with greater confidence.

What is medication management?

Anytime you’re taking prescription or over-the-counter drugs, it’s important to make sure you’re using them safely and effectively. You may benefit from medication management services if you:

  • Manage multiple health conditions
  • Have a chronic condition, such as asthma, COPD, diabetes, heart disease or high blood pressure
  • Recently had a hospital stay
  • Take multiple prescriptions

Our clinic pharmacists work collaboratively with your primary care provider to identify the best treatment options for you. We spend time with you to provide education and answer questions about your medications, in-person, by phone and through Telecare visits.

We can also help with topics such as tobacco cessation, how to monitor blood sugar and questions about blood pressure.

All of our primary care clinics offer medication management services, and providers at these locations can provide a referral for you to meet with a pharmacist.
